Whenever I'm at home or work, people are like " Are you ok? You don't look ok " or things like " You look upset whats wrong? " I really don't get it.
Old Italian saying: If 3 people tell you the same thing, believe it. Maybe you should see your doctor for a checkup.

in my case, I lay down on the desk (sometimes friends/someone asks same thing to me)
I just started at a new restaurant and one of the bartenders keeps asking me “are you ok sweetie”. To the point where I told her the other night “hey I’m ok. This is just my face. I promise I’ll let you know if I have an issue”. Jokingly of course. But fuck, that shit is annoying.
That is usually based on your skin tone and/or body language.
People that have certain health issues (some very serious) have a skin tone change. One of the best examples I know of is that Larry King (a once popular talk show host - who died in January) decades ago did a live interview with a past US Surgeon General Dr. Koop.
A year later he told the story that as soon as the 10 minute live interview was over that Dr. Koop told him that Larry King should immediately head to an emergency room as he did not like the skin tone that he was seeing. Larry King had a staffer drive him to the closest emergency room and it likely saved his life that week as he was having the start of a significant heat attack. Larry King lived for about another 30 years after that and just died at age 87.
Please pay attention: If many people are telling you this something is not right - and your body is clearly showing it. Be it diet, medical, psychological, or a combination of those things.
