You're not as fucked up as you think, your not a sicko, and there's no reason to feel guilty. People have fantasies all of the time about things that go completely against societal and moral grain.
The key is to not act on it. You may be powerless over your desires/fantasies, but not your behavior. May need some help, though, managing the fantasies in order to assure that you don't try and act on them
It's normal. Most of the people here who say you need help have similar fantasies, and prefer bashing people like you instead of telling the people they fantasize about. I suggest you tell her, instead of hiding it, and then ask her how she feels about it.
I think your fantasies are more common that you realize, like the above commenter says "don't act on them" enjoy them, even when your with your lover, but never tell a soul or you will ruin your relationships. Sometimes, siblings know or sense these things.
