Is it normal to go walk through the hospital in a grim reaper costume?

That would be mean. I would advice you not to do this. Yes, it would be funny, but you actually may scare an elderly person badly enough to cause a health problem or worse! Then you could be held liable, and could be sued!
I'll tell you what, I work in a hospital, if you try to pull that bullshit, someones going to call a code on your ass and security will swiftly escort you OUT. We have many members of security in the hospital due to individuals who are not completely and mentally there. They get violent or confused. Hospitals are full of elderly people. Very rarely will you find a young person who is going to get your "Joke." You would only be hurting the already confused, mentally unstable, injured and generally unwell people that are already unfortunate enough to be in the hospital. Not even you will find that prank funny when you get reamed out for being the insensitive jerk who gave an elderly person a heart attack. I strongly advise you never to go through with this.

This would be the same as me dressing up as an employment agency executive and walking the halls of a liberal arts college.
It would be funny if somebody got so mad that they killed you on the spot.

Hahahahahaha!!! I would prolly piss myself if I ever saw this. But, I wouldn't get a proper scythe as it may get you arrested and people could turn out a lot worse.
