If your friend has never experienced a life any different than the one he currently exists in, how could he be motivated to do anything else except what he knows.

People say trite shit like "why don't they just eat less and exercise more". It's not that simple and it doesn't take into account other factors that contribute to an obese person's situation.

For example, they are constantly ridiculed by people when they go out in public, so they stay at home where it's safer. Their excessive weight puts an enormous strain on their joints and ligaments making it excruciatingly painful for them to exercise. They are not included as a part of normal society due to people's negative attitudes towards them, so they don't have the same social opportunities that everyone else has. Food brings them a lot of the comfort that social relationships bring others, even though it is one of the things that is also holding them back. They are constantly bombarded with advertisements about food (especially junk), but unlike fit people who are able to tune it out, food is their vice and they are hypersensitive to those advertisements.

So, these obese people stay at home to avoid the shitty world that hates them so much, consoling themselves with a pint of ice cream.

Who are we to judge them so harshly, especially when we've never walked a mile in their shoes?
