I mean that he is a boy but he wants to be a girl. As his friend, I support his decision and freedom.
You can't change your sex. You can cut your genitals off and you can pretend being a woman, presenting yourself as a woman, telling yourself you are one as long as you want but it won't change the fact that you're just a castrated man. You can't change biology. You can only change the surface.
It's normal that your friend trusts you enough to confide something so personal in you if you're a good and trustworthy friend.
