When I was younger, I sometimes used to play with a female cousin. We've since grown apart, but sometimes I fantasize about having sex with her, and if I find porn of girls that remind me of her, it turns me on like nothing else.

All it means is that you feel sexually attracted to your cousin, and it's more common than you think. I don't think it's okay though, she's your family, when you see her, try to think of her that way.
Sorry, but it's easy to be thrown off by his choice of words -- he says he would "play with" her; that would suggest that they were very young at the time.
Furthermore, he says that they've since grown apart -- that would suggest (in my mind) that he remembers her as she was back then.

No, and I'm not sure what gives you that idea. She was only 1 year or 2 younger than me, and we're both well over 18 now.
