Is it normal to feel like a failure?
I constantly feel guilty and on-edge, like there is more I could be doing and I'm just being lazy. I recently got accepted into a PhD program and people tell me I work hard, but I constantly feel like a failure.

I hope you can get to a point where you're kinder to yourself. Anxiety sucks.
If you still feel like a failure after getting accepted into a PHD program then probably no amount of success will help you feel like a success. The only thing you can do is to realise that you're not a failure, and to take pride in your accomplishments. Maybe you should talk to a therapist or something?
If you're doing a PhD you're DEFINITELY not lazy and you have a lot to be proud of.
Some people do have anxiety about time, more specifically they can feel like they're not getting enough done within a day or by a certain point in their life. Sounds like you might have that.
