The only way I can achieve orgasm is by going to restrooms and finding toilets where people haven't flushed after pissing, putting a rag in the toilet water and holding it against my mouth and nose while I toss off. I only do this in ladies rooms, so I have to be very sneaky (I am a guy). I will often wait around the mall and watch women go in the restroom and check the stalls after they leave, then if I find that they left without flushing I'll think about them while doing my business. I told my family about this and they are giving me grief about it, but I think it's a relatively normal "fetish".

    I have a feeling that this isn't a totally real scenario.
    But just in case it is, there are a few points I would like to make. Having a paraphilia, AKA a sexual fetish, is normal. However, it could be argued that the sexual behavior you partake in is not. It's definitely not a healthy release, at least in my opinion, as you're taking a high level of risk. First off, there's the risk of getting kicked out of whatever public place you're in and possibly arrested for something along the lines of criminal mischief or the like- not something you would want on your record. Second, a public toilet is very well-known to be a rather dirty place. You're taking a major health risk in consuming that toilet water. I'm not too knowledgable about the spread of disease through urine, though if that water is contaminated with fecal matter, semen, vaginal fluid, saliva or menstrual blood, then I'm fairly certain that you'd be putting yourself at risk for possible transmission of STD's or other infections. Don't get me wrong; I'm absolutely NOT judging you or labeling you as a freak, as that's simply not how I see the situation, but I do strongly urge you to take serious precautions, if not entirely curb that particular behavior for your own safety.
