Men's testicles need to maintain a certain temperature to remain healthy and fertile, because that's where all the sperm is and the reason why it hangs from the body. Men's underwear are design with that purpose in mind, from men who like a little more room with the boxers, or for those who want it a little more snug with briefs and such, and they accommodate the different sizes since all men are not the same.
