Good luck if it's old blood. I've had this issue many times with period blood on sheets and stuff. You're stuck with a stain forever.

If the menstrual stains are fresh, washing the spots with hydrogen peroxide helps.

If the stains are old and already set in, you're stuck with them. Sure, vinegar could help fade them, but it won't get them out completely.

Btw things like hydrogen peroxide and vinegar will compromise the integrity of the fabric, so use with caution.

For this reason I like to use cold water and salt, then warmish (but not hot) water and bar soap.

I've found that using some sort of stain remover (something like Spray 'N' Wash, OxyClean, etc.), allowing it to sit, and then washing usually does the trick. It works best if it's fresh, but might still at least fade it if it's older.
