I rarely use them but they're usually clean or at least acceptable in my experience. 75% clean-acceptable to 25% gross.
One time last year I was in K Mart and I went to the bathroom and pushed open the stall door (it was not locked and was slightly ajar) and there was an old woman in there with her shirt pulled all the way up and her pants all the way down. I turned around and left but I kept seeing her around the store as I was shopping. It was awkward.
