Well it doesn't cause immediate acute psychosis. Herioin itself isn't all that bad....lots of the ill effects are not from the drug itself but things like sharing needles, etc. There's tons of people who use prescription opiates, which are far stronger than heroin, and they get by fine, you couldn't even tell who these people were unless someone told you. Meth, on the other hand....
I used to live in meth central, and every day you'd hear these crazy stories on the news about some weird shit meth heads did. There was one story that stuck in my mind, a couple were all meth'd out and took apart a TV and were trying to use mercury to get the gold out of the circuitry of the TV. Well, they died from it....but wow.
