If it is actually Restless Leg Syndrome, she has no control over it. Do you habitually get annoyed at people with disabilities or illnesses which cause you any inconvenience?
If it is just a nervous habit of hers, then maybe she doesn't even realise she's bumping your desk.
Why the hell would you need to write a note? I understand that you don't know her well enough to have her number, and so you can't communicate by the only means most young people are comfortable with these days, but why the hell can't you just look her in the eye one day before or after class, smile, and say, "Excuse me. I'm sorry to mention it, but..."
