I think the whole point he was making was if EVERYBODY waited til marriage to have sex, and only had sex with their spouse, then STD's wouldn't be a concern.
If, since the beginning of time, people ONLY had sex with their spouse, STD's as we know them wouldn't have developed. The opportunity that these diseases take advantage of wouldn't be there so they would have developed to transmit by other means just like any other disease.
If the only way to get a disease is by blood transfusion, then it's not exactly an STD now is it?
It's still an STD regardless. STD's can develop within animals other than humans and cross contamination can occur.
What defines an STD is the possibility of passing it via sexual contact so even if it's not being transferred that way, it is an STD.
Also, if people only had sex with their spouse since the dawn of time, there's a decent chance none of us would be here today. There were several bottlenecks within human history that if polygamy didn't occur, humanity may have died off. It's a sad truth.
