Because gender is about identity, not anatomy. Men who were born with penises are called cisgender men (or cis men), while men who were born with vaginas are called transgender men (or transmen), but they're all real men. A transman is different from a woman who dresses as a man (even if he has a vagina), because a woman who dresses as a man (a drag king or cross dresser) is doing it temporarily for fun or for a performance. She doesn't identify as a man, and when she takes those clothes off, she's still a woman. She's dressing in a way contrary to her gender identity. A transman, on the other hand, isn't playing dress up or pretending; he's dressing in a way that's consistent with his gender identity.
Also, many transmen undergo hormone therapy so that they're hormonally identical to cis men and/or surgery so that they have penises. Hormone therapy for transmen can shrink breasts (though some men choose to have reduction surgery as well), broadens shoulders, deepens the voice, increases muscle mass, stops menstruation (in men with vaginas), and causes facial hair growth. In these ways, transmen who undergo hormone therapy are anatomically very similar to cisgender men.
I don't think that transmen think "I would like to be a transman", either. From what I've read, it seems like they think "I would like to be a man/boy", like you do. Of course, it's not my job to tell you what your gender identity should be, and I'm not trying to do that, but I hope that this clears up some misconceptions about transmen.
It is helpful but I want to be what you call a cisgender man, if I understand correctly that is a "real man": no cosmetic procedures necessary... Trans people confuse me. There are only 2 real sexes, but I understand gender is a whole other matter. I don't want to be a "man with a vagina" or fake penis(? I didnt know they could do this)
It's not a fake penis, actually, it's a real penis that they surgically construct using your actual tissue. I don't know everything about how it works, but yes, they can absolutely give you a functioning penis.
And I think when you say "real man", you mean "cisgender man", but you should know that if you refer to cisgender men as "real men", some people will find that hurtful and offensive (especially transgender people).
