As others have said, having intrusive thoughts about harming a person or creature we care about (or yourself) is something that most people occasionally experience.
We've all got a very dark side hidden away down deep in our heads. Thoughts from there squirm their way up to the surface now and then, and it doesn't mean you're going nuts or need medication or psychiatric help.
I suggest you just acknowledge the thought, tell yourself firmly that it's something you'd never do, and move on.
In any case, what you thought of really wasn't all that ghastly, since many breeds of dogs used to routinely have their ears clipped. Maybe some memory of hearing or reading this popped up out of your subconscious and prompted the thought. It's not like you suddenly had the urge to take the scissors, disembowel the puppy, stab him in the eyes and pin him to the wall with the scissors.
