Getting people's names wrong on purpose is pure passive-aggressive behavior. You're right; she's rude as hell. Unfortunately, none of your options for dealing with it are great. You can just deal with having a jerk boss -- lots of people do -- or you can risk her animosity by correcting her on your name again. You can get in touch with her supervisor or with your company's HR department if she's really obnoxious, but that doesn't always produce any measurable results. You can look for another job.
