There's a key statement everyone seems to have missed. The boy approached the male twin to ask if he was going to bathe him and cuddle afterwards. That sounds to me as if the boy is gay. If so, I doubt he got much stimulation from the twin sister's activities. Here's what I suggest:
1. Tell your sister that what she did is wrong, that there are to be no compromises, and that you will report her to YOUR parents (not the boy's at first) if she does it again.
2. Ask the boy casually whether he prefers that your sister babysit him by indicating that you may start dating more. But tell him you will try to rearrange your dating schedule if he wants you to continue babysitting him. That should give you a bit of an idea of whether he really wants your sister or you which will be a bit of an indication of how much he liked being bathed by her vs. how gay he might be.
Has the boy ever asked you again if you would bathe him?
