In Spain we have universal healthcare. Any specialty care has waiting list. You don't get an MRI or CT scan unless you're on deaths bed. Since it's government controlled it's considered too expensive and waiting list are too long. Any rehab or care like hospice for end of life is left up to the families. Doctors are on a government pay scale and most could care less about their patients, it's kind of assembly line medical care.
There is still at least two levels of care. If you are rich and can afford it, or high up in certain businesses you get private care. Even that is divided by how rich, the best doctors and care is all paid out of pocket and very expensive.
The final point I'll mention is in the USA, everything is ran by politics. At some point I can see people on the far right or far left not necessarily being refused certain care but definitely making it harder for some to find or get care.
Until USA figures out how to keep politics from influencing everything and changing every couple years. Anything and everything ran by your government is not good to count on long term. Think national defense Dems always want to cut and have less, Repubs always over fund and increase.
If defense is not a good comparison for you, think your medicare and social security system, they've been around a long time but each year are influenced by politics on funding and what they will and will not cover.
Also darefu you seem to forget here merica if I was in charge of hospital lets say you come in for just a broken finger I can charge you whatever I want for the scans and procedures that more than likely your insurance will refuse to cover because hey that money you give to the insurance company is supposed to cover that right? Nope most insurance companies will flat out tell you to go fuck yourself and now you gotta pay outta pocket for that unreasonably high hospital bill. Oh btw you want an ambulance ride in that emergency you can't control too bad bud you gotta pay that to.
From what I remember in the US you have various plans even under the ACA and Medicare. You have to read and understand. Some cover certain things some don't. Some cover those things only if provided by certain providers.
That's the same under universal health care. If I break my arm and go to the wrong hospital or doctor I'm responsible for the bill. I'm required to use one that belongs to the state system even if it's 5 miles away and the other is next door. Only time an exception may apply is in life threatening conditions.
Ambulance service is the same I could be lying on the ground next to one but if they are not part of the state system they've not taking me, or if they do I'm going to the hospital they're affiliated with. The bills will be mine.

What do you think happens if you have cancer and can't get tests or treatment for 6 to 8 months. You die! What do you think happens when you are lying on the ground after an accident (next to an ambulance) but they can't take you because they are not part of the state system. You die!
If you do elect to take that ambulance they will only take you to their hospital which is not part of the state system. The good you are probably going to get great care, the bad you are going to be bankrupt. Which is not the same as bankrupt in the USA. It means you still owe the bills you just get appointed a money manager. You get an allowance and they get control over your money, and where and when you work.
I've seen both these situations happen.
My comments are not in support of the rich, they are just to point out, that if you have a universal health care system, everyone is going to pay through taxes and you get little as well as poor quality care in return.
I'd much rather pay once and have a choice of where and who gives me care.

Not necessarily too expensive for what I can afford, but too expensive for what I get in return. 500 euro a month, to sit on a stationary bike, do a few stretches, an ice pack, and electric stimulation. All done in 1 hour, 2 times a week. That's a government program for you.
If you think government programs are free think again. You just don't know what your talking about. Either that or you just want a free ride and don't understand, you don't get something for nothing.
