Yuck, there's nothing like a creepy inappropriate girl to ruin a friendship and sometimes a relationship. At least she is, as you say, not very pretty. However, it's not uncommon that the stereotypical unattractive person is willing to go the extra mile for the attention he or she desires.
I can tell you from experience that the woman my first ex husband cheated on me with wasn't a looker. In my humble opinion the fact that she's telling lies about you to your boyfriend should be a dealbreaker for both him and yourself. I also think the both of you ought to present a united front and phase her out as a friend. I think it would be great if your boyfriend would be willing to block her number. I wouldn't necessarily be in a hurry to confront her but should you have to I would advice that the both of you tell her that ya'll need a break from her.
Hopefully she will get the message.
