I use my best judgement. If I haven't washed my hands much that day, they're dirty or I'm about to eat I'll wash them.
But its not like they're dirty from going to the bathroom.
So many people wash their hands only to pick their nose or sneeze in them five minutes later. I always carry a waterless cleanser, but who are we kidding? We're teeming with bacteria anyhow.
