I've heard of something similar before. An uncontrollable urge to hit yourself could be a form of OCD. If my knowledge is correct, though I have no qualifications to back me up, it's not a form of self loathing at all, it's more like a mental mis-fire, it manifests differently in different people. If it's simple enough, like touching your nose, you can probably laugh it off and try to blend in- hitting yourself and leaving marks is a bit more problematic. Having this problem reapear in college makes perfect sense - it's s high stress time and increased stress agravates OCD. With medical help, often drugs and/or therapy this can be overcome.
