I've had testicular cancer, complete w/ orchidectomy and chemo. It's pretty easy to detect lumps or a full-on tumour just from self-examination - no feel-up from you necessary. If he genuinely feels something down there he'll need to get referred to a urologist anyway.
Btw a tumour on the nuts is almost always malignant.
