Yes, I actually witnessed this, and it wasn't just one or two people, I saw about seven do it.
Oh I've done a few things that, while not the brightest of ideas, still turned out pretty well even though it really could have been done better in hindsight. The difference is that I learn from those mistakes quickly and do my best not to make it a habit.
Then again I am a lot harder on myself for my mistakes than the average person seems to be. I don't see my mistakes as acceptable, I see them as a learning experience and do my best not to repeat them but I never see them as acceptable nor are they excusable. I really hold fast to the belief that one should live and learn but when people refuse to learn it can be a bit of a problem.
