As far as I'm concerned, as long as a fetish doesn't affect anyone else in a negative way, people should be free to do whatever the hell they want in private.
Acknowledging and accepting ourselves for who we truly are is definitely much healthier psychologically than staying stuck in a pigeon hole that doesn't fit us just because others have told us that's where we belong.
There's a moral grey area when a fetish involves self-harm or the person with the obsessive interest actually risking their life. That raises questions about their mental health and that in turn raises questions about how responsible we are for protecting others from themselves. But, in general, I believe people should have the right to damage their own bodies or do things that either could or definitely will end their own life if they want to do that.
