i think its normal, sometimes under stimulated people need that adreneline if hearing screams or whimpers. i should hope the pain is not from excruciating pain, unless your a domme and they are a submissive-in future circumstances.
i too, thought of hitler only because you made a point to mention the religion or ethnicity of her for example " i was playing the doctor ( control) and the little black boy was playing the patient (vulnerable) i loved hearing him scream?! bwahahaha.
