It's called emetophobia. I suffer from it as well, to a certain degree. Although I'm still pretty young, I've found ways to deal with it. Places like amusement parks still freak me out though. I can't go on roller coasters without wearing a big jacket, for feat of someone vomitting on me, or even near. But the jacket doesn't stop my crazy panic attacks(which is what happened to you in fifth grade).
Think back to your childhood. Is there a particularly traumatic vomitting experience you remember? Maybe something completely out of the blue? I'm thinking there was something more than just the fifth grade incident, since you had a panic attack at that time.
Sometimes, as gross and revolting as it may seem, it helps to see photos of vomit. At least then, you know it's not near you, and there's no chance of hearing, smelling, or touching you. It was one of the first things I did to begin overcoming my fear.
