I have a friend who was a casual user and now doesn't use at all. It is entirely possibly to use heroin casually. However, the reason people don't say this is because it encourages others. Everyone thinks they can be that casual user, that addiction is something that happens to other, weaker people. The fact that heroin addiction is commonplace shows that many are wrong in that judgement.
