If you have E-coli than I think anal sex should be the least of your concern.
Obviously you have to have e-coli already for that to be an issue with anal sex since it doesn't just magically appear in a healthy person. Your partner could possibly becoming similarly infected from it if you already have e-coli. Though to be fair your partner would also be at risk if you cooked for him or made out with him if you have e-coli.
Unless you are talking about some sort of ass to mouth anal sex orgy with multiple partners then yes hepatitis A and e coli spread would be a concern. If you're into that then yes, you really should medically test participants (though really that goes without saying).

There are harmless forms of E. Coli that indeed are in the digestive tract of most healthy people. There are also harmful pathogenic strains that form what people colloquially refer to as E. Coli. I think it was pretty clear from context that we were referring to harmful pathogenic strains.
Though I will note in future I should say pathogenic or harmful to be more clear.
