i had a baby last year...this was one thing i was soo worried about. this and pooping during delivery - omg i would die. my doc's practice wouldn't do elective c-sections but my baby's shoulder's measured pretty wide on sonograms and they said she wouldn't fit so i had a c-section anyway. i don't have much of a scar and even though recovery was longer i didn't have that cut or rip deal to worry about and my lady parts are still tight like before so everything is still very fun ;) with big babies it's easier to have a c-section. i was extremely glad i went that route. plus they are planned in advance so you can usually avoid last min rush to hospital or someone important not being there.
