i wonder that too. just squat beside the car, possibly with both front and rear doors open to create a kind of "cabin", and pee off the road so the others can also use the "cabin", or move the car a couple feet for a new "cabin".
I mean, be creative, you don't have to pull over and then undress and pee standing in the middle of the road.
Is it normal that I go outside?
