It's totally normal to be a bit confused about whether or not you should kiss her, and I find it sweet that you're so worried about not upsetting her. Based on what you've described, I think she would probably be okay with it. I'm nervous about initiating a kiss with my boyfriend (same situation as you: first boyfriend, still a bit nervous), so I'll usually just put my head on his shoulder or chest, or move a bit closer to show that I want him to kiss me, which is almost every moment we spend together. Your girlfriend seems to be doing a lot of the same things, so I'd say go for it! :)
As for how to initiate, try just bringing your faces closer, and maybe gently touch her face, then wait a second to see how she react. If she doesn't move away, she's okay with it and is giving you permission. If she herself closes the distance...that's obviously an acceptance. If she does move away, though, just smile and accept it. Most likely, she's just nervous and it has nothing to do with you. If she's comfortable cuddling, though, she's most likely okay with kissing.
