"I can't fathom why on earth anyone would want to be a different gender." That's it right there. That same conviction, that certainty that you wouldn't want to be anything other than your own gender -- that's exactly it. That's the reason trans people endure the pain, the humiliation, the suffering, the loss of rights, family members, jobs, housing, dignity, the violence. Nobody would voluntarily choose that. Because the choice is between all of that and living in the wrong gender. (Or, unfortunately, for far too many trans people, not having any livable choice at all.)
