I feel the same way about relationships. You just gotta be honest with him and with yourself. I've made it clear in my relationships that I cannot handle long-distance and that if it comes to that I'd either find someone else to be my emotional support or we'd have to break up. Maybe having it clear like that will make him reconsider his options...like going to a school that's close by or making an effort to see you regularly despite the distance.
If you don't say anything the relationship will be over in a year anyway, so what you do depends on how much effort you're willing to put into this relationship to make it work in the long-term.
I think it's actually cute that she cares so much. Even though it would probably be kind of an annoying ad unexpected turn of events for the BF that she feels this way it is always better to share these feelings. IMO if after this disclosure from whom I assume he considers the love of his life he really doesn't seem to be willing to make an effort to change or at least help to improve the situation he wouldn't be the right guy anyway..
