It is kind of ok but from personal experience when I was already in teen years with functioning boy-part down there my mom used to come with me to health checks as well. Those health checks didn't involve pulling down panties and visually examining genitals but later on I did realize that my panties didn't fully cover my crotch from the side angle as in testicles were visible (was wearing classical briefs back then) and that realization made me feel a bit embarrassed that my mom and female doctor saw this.

It is fundamentally different when the mother planted her presence in advance while the upcoming event was known to her son to the smallest detail thanks to previous visits. Taking advantage of the medical office policy and her parental authority, she can enter the exam room with her son and take a position from which to have an unobstructed view of the whole procedure. And this usually included genital check at the end of the exam where the doctor pulls the underpants to boy's knees and conducts hernia and testicular examination and retraktion of the foreskin for check for phimosis. Practically, due to the small size of the exam room from such a distance she can see just as much as the examining doctor. Although I exclude any sexual aspect or any perverse interest of the mother to the naked body of her son, this presence will inevitably cause discomfort and shame in the mind of the developing young man. And maybe a certain amount of excitement too?
