Not everyone uses gags in bdsm but I understand your point. When gags are used a verbal safe word isn’t used but there are other ways of letting the dom know they are ok. Blinking is a common alternative when words are not possible.
Hand signals work but eye movements or blinking is best. Wouldn't recommend doing bondage plus gags on your first few times. People should get comfortable with their partner and their limits first.
