If they're demented and they're just staring blankly at a wall or the side of a car, that seems cruel, but if they're not really home any more, they just don't care. Maybe the idea is that they'll enjoy the sunshine?
If they're not quite that far gone, maybe they enjoy watching traffic going by and getting some fresh air?
Some nursing homes are good, but others are ghastly places, run by people who care only about money, and staffed by anyone who's willing to clean shit for minimum wage. So it is possible that what you're seeing is a form of elder-abuse. It wouldn't hurt to find out who's in charge of licencing nursing homes in your area, and giving them a call to express your concerns.
