I don't want to insult anybody here, but personally, I wouldn't be able to. Infact, I find menstruation so gross and unhygienic that I went to great lengths finding a birth control pill capable of eliminating my periods completely. Parts of your underwear still touches your skin, and skin sweats and gets rid of dead skin cells that attatch to the clothing, so it would be wise to change them even if you don't feel like they're dirty cause trust me, they probaby are.

Which pill?
Great idea, I used to do that when I was younger. i don`t understand why all women don`t do this.
