He's sickly too, don't you forget. Even at the start of the story he has a history of life-threatening illness and a limb amputation. It isn't like Augustus and Hazel occupy completely opposite ends of the health spectrum :P
I think the traditional Hollywood Love is what is unrealistic. I appreciate the story of The Fault In Our Stars precisely because it's realistic, insofar as it approaches human emotion unironically and directly and plausibly, with all the melodrama and exaggeration which characterize teenage love. The characters accurately portray what a real teenage relationship looks like, because the characters use the same logic that real teenagers do.
