I agree with previous posters - it's probably a relatively common physical problem that might, in an extreme scenario, require some surgical intervention. But there're so many different things that can make vaginal penetration difficult for women - go ahead and ask your doctor about it.
For example, some women develop a problem called vaginismus where, try as they might, they can't relax their vaginal walls enough to put anything up there. It's technically classified as a psychological problem but I think of it more like an unpleasant reflex - I mean, why wouldn't your body try to prevent you from sticking stuff up there? It can feel really weird, especially when you're not aroused.
I don't mean to imply that you have vaginismus - more likely, you're just super tight. In any case, if you do ask a doctor, I guarantee it won't be the first or last time that someone asks them about tampon insertion.
