I thought a guy was crazy but he had a heart attack and died

I saw a guy on a lonely beach near my house laying on his belly moaning loudly in a really funny way. It switched from sounding sexual to a relieved "ufff". I thought he was super drunk and having his fun and told my friend to come watch. He suggested to film it.

I wanted to go for a quick dip and asked my friend to observe, as I was scared the "crazy" man was potentially going to attack me at that lonely beach.

After 2 min of swimming I went to my house to take a shower when my friend was approached by the guy's wife who asked him to call an ambulance. She and her kids gathered around the man who apparently wasn't crazy or drunk, but having health issues. The man was very adipose.

30 min later a doctor arrived who insulted the guy's wife and kids for not doing anything. He started an actual fist fight with our neighbor (who had called him, because the ambulance took such a long time to arrive). The guy was still laying in the sand face down, moaning.

They were able to help him walk up the stairs and he looked confused, but not entirely out of it and could answer questions. He had apparently swallowed some salt water while swimming.

The ambulance arrived about 80 min after we had called it and took him to the hospital.

We were informed that he had had a heart attack and died in the hospital.

I now feel responsible for his death as I didn't realize he was suffering and mistook him for a drunk, crazy person.

How would you have reacted? Why didn't he call help, when being able to answer questions? Am I a terrible person?

    This wasn’t your fault. I would have called an ambulance either way drunk or not. But can’t blame yourself when you didn’t know he was suffering. His death isn’t on your hands. Just the next time you see something like that call either way. That’s all you can do.

    youre saying the man's personal doctor was called to the beach to start throwing punches at someone for no reason? and then they let you know as random strangers, that the man died later on? sounds bullshit

      It was just another „emergency“ doctor on that island. Wth he was fighting instead of helping that man was just insanely ridiculous.
      The guy who died was on vacation just like us and his family came by at night and informed everyone involved.
