You go up to her and tell her you're really sorry you rejected her, you realise you had your priorities all wrong and you want to go on a date with her somewhere. There's no need to over-complicate it by over-apologizing for rejecting her or anything else, just say that. If she's still interested, she'll go out with you. If she doesn't, then at least you did all you could.
