You are thinking way to much about this. People get use to calling people by something, a nickname and do it without even thinking. Nicknames can start from anything. In hs we had a dude we still call ass muffin or "muffin" to this day just because one time he sat on a chocolate muffin with his white pants and it literally looked like he shit them with explosive gooey diarrhea.
