You shouldn't use cotton buds (q-tips) inside the ear, only to clean the external parts around the opening. It can be dangerous and counter-intuitive to push a cotton bud inside the ear canal - but for some reason (particularly in the US) it's a bad habit passed down from generation to generation.
If you have a painful build up of wax within the ear it's advisable to visit a doctor who can use a syringe; the syringe doesn't suck the wax out, instead it allows a few drops of liquid (commonly hydrogen peroxide) into the ear which loosens the hard wax allowing it to trickle out when your head is tilted. You can in fact do this at home, which might be advisable if you're prone to build ups, but make sure you follow proper instructions.
I have never cleaned my ears with a cotton bud. Whenever I have a shower, the water tends to do most of the work in cleaning my ears anyway. I can hear perfectly well and have never had a build up of wax or indeed any problem at all with my ears.
I never clean out my ears.

Very true, it's amazing how many people think the opposite about "ear hole care".
