When you have a baby with someone in your family, it increase the chance a recessive gene is paired with a copie of itself and thus express. It can have bad, but also good effects.
If it's mostly bad effects, you are unlucky, BUT your kid is going to die or have no baby and the bad gene end there. If you did a baby with someone outside the family, the bad gene continue his trip as it's passed from parents to half the babies and it will maybe en up with more broken babies when 300 years later your descendant meet the descendant of your brother.
Children of incestuous relationships, assuming it's the first in the family line and not multi-generational, needn't produce kids with disabilities unless there's a genetic inheritance being passed on.
