I would disagree with labelling of any of those conditions that you previously outlined as fundamentally delusional though. There are often physical problems relating to ED, some people with autism are quite genuinely and obviously suffering from a mental illness, people with depression often have crtical levels of seratonin, etc, etc
I would say that absolutely a transgender person should be able to do what they want. If they want to slice off their testicles if they feel like a woman, or stick their hands in dry ice until they fall off if they believe they're disabled then they should have the right to do so, even otherkin should be able to get surgery to make them look like a wolf or whatever if they want to, that's freedom.
I just don't think the way to "help" with what they're going through, by and large, is for society to reaffirm their identity, and if someone asks me for my opinion on what do in one of those situations then I would advise, as I did, not to head down the path of non-reality.

So basically, the loony toons who are not really TG cannot easily get such a drastic surgery.

Idk man, I didn't want to be rude, but you know when you go and see, lets say, your uncle with Alzheimer's, and you realise he's just to far gone to fix so you just agree with him that aunty Jill is still alive and the just won a bunch of green on the ponies? I think for some of them they're essentially beyond the realms of rational treatment so the best thing to do is just lie and agree with them when you're around them.
