many cousin marriages lead to healthy babies and it's a lot more common in the world than you realize. In terms of reproduction the incidence of defects is based on how close they are in the family tree. So a brother and sister would have a higher likelihood of genetic defects than cousins.

people hook up because of their intense attraction for one another and their shared history. They don't go looking for it, it just happens and they're not in it for pumping out 17 children (though some crazy cults do such things).

I think you'd be surprised at how many people get little crushes like this on extended family. Also the rate of birth defects from interbred first cousins is surprisingly low. (But I still think, like you, that actual sex / reproduction would be icky)
