Weed can actually cause hallucinations, it's just extremely rare. It means that you're very sensitive to it. Maybe it's wise not to try LSD? I'm not sure if that would be related.
It's really not that rare. It's happened to me plenty. The second time I got high, I lost all sense of superposition (which according to my psych professor is not even possible under any circumstances.) It's just a question of how much you smoke at once, how strong it is, and what kind of tolerance you have.

Not all that rare. Just means you smoked some really dank bud!
