In a typical man, the urethral sphincter (the valve that controls the release of urine from the bladder) closes tightly when he gets an erection. This blocks off the part of the urethra that runs from the testicles to the penis, so it prevents his semen from going into his bladder when he ejaculates.
Most guys find it very uncomfortable if they bear down and try to urinate with an erection. I suspect the instinctive reaction of most guys when they try to do it is to worry that the discomfort and feeling of restriction means they're doing something that might damage a particularly precious part of their body, so they don't persist in trying. But people vary, and some guys apparently don't find it completely impossible.
If a guy has a painfully full bladder and he can't urinate due to a very firm erection, the obvious solution is to get it to soften a little. Getting away from the object of his lust, splashing really cold water on the penis and thinking very unerotic thoughts might help with that. Sitting down on the toilet might also help since it shifts things around internally. But forcing a penis to bend is never a good idea (penile fractures are a real thing), so getting the business end of the penis aimed down into the toilet below the rim can require the guy to sit in a very odd position.
