I do think it's out of line and I would definitely call someone out if they said somtething like that around me.
People with disabilities are either born that way or suffer some horrible accident which causes it. To blame someone on something that was due to circumstance is not only ignorant, it's dehumanizing.
We're all just people, regardless of any other factors. Seems to be one of the biggest things we forget though...
Don't feel bad about it, at least you're tactful. I'd probably be better off if I learned to keep my mouth shut more, been kicked out of a few places for speaking my mind when it wasn't appropriate.
You're not obligated to confront anyone. However, if she thinks it's appropriate to say something that awful, replying "fuck you" isn't going to lower the quality of the conversation.
